FFS rog maybe I just don’t understand, but with the title and the op I guess the question was raised if a single 45 minute break taken at the 6 hour mark would be legal for a 15 hour shift. The answer is a resounding NO. If you then start to add POA into the fray and other complications that you seem so intent on then maybe.
If you want to be an arse/pedantic the max day that can be worked is 21 hours.

It says “Working Time Breaks and 15 hour days”.
A 45 minute break at 5 hours 59 minutes will not legally “count as your full wtd break for the day” (comprising a 15 hour shift) unless there’s at-least 2 hours and 16 minute of POA/break in the shift and after the 45 minute break.
As the OP never mentioned or asked about POA, or breaks shorter than 15 minutes, it seems to me to be reasonable to assume that he will need to have completed another break of at-least 15 minutes sometime before exceeding another 6 hours working time.
Therefore the answer to the OPs question is, No a 45 minute break at 5 hours 59 minutes will not “count as your full wtd break for the day”, you will need another 15 minute break before exceeding another 6 hours working time.
For a 15 hour shift it could be legal - it all depends on the working time in that shift
The driver might do drive 4 hrs + other work 2 hrs followed by a 45 break
Driver then does 4 hrs drive + 2 hrs other work + 2.25 hrs POA
