Question is, If you work five days from Monday at 5 am and you only do a 13 hours duty day once, the rest are over 13 hours, explain how you could get 4th reduced day in.

You can’t. In that scenario there is not enough time for a reduced weekly rest of 24 hours, which would reset the reduced daily rests available to 3, and a split daily rest of 3+9 is not a reduced daily rest so it is impossible to get a 4th reduced daily rest in.

You don’t really need back-to-back weekly rest periods in that situation unless you want to be able to go beyond 23:59 the following Sunday before you begin the next weekly rest. You can begin a 45 no later than 05:00 Sunday and you have covered the 144 hours between weekly rests and taken a 45 at the same time, which is what your question wanted. Or, and still legal for the 6x24 and getting a 45 in the correct time frame, you could take a reduced no later than 05:00 Sunday and then begin a full weekly rest 144 hours after that rest ends, or 23:59 the following Sunday, whichever comes first.

If you really want to go beyond 23:59 the following Sunday you could do a 69 hour rest beginning no later than 05:00 Sunday which would give you 144 hours from the end of that rest period before the next was required and if you do start it at 05:00 Sunday it would take you to 02:00 a week on Tuesday before the next weekly rest was due.

Where did the weekly rest issue come from ?

Oh, I have just seen it - its the way the question is worded - I assumed the question was aimed at getting in an extended duty day rather than a 4th reduced daily rest

Why would you assume that? There is no such thing in the regulations as an ‘extended duty day’, in fact the duty length is never specified in the regs, so I don’t know why you would assume something that isn’t there.
