The more up market ones come with an armrest on the iside of the drivers seat.

It cost £100 in diesel to do 640 miles that seems ok to me considering there was 14 people and luggage in it i have no idea how many gallons it was so i cant work out the mpg.
On a rough calculation, it’s about 27mpg which sounds OK. I regularly drive a Sprinter with a Luton body and that usually manages around the same.
