This happened to me week before last…I rang the local VOSA office and spoke to a agent…then rang DVLA they sent out the form D777B. The good thing about a malfunctioning card is it’s FREE.
Also the VOSA agent said just do a printout at the end with your signature…the VOSA site states a printout at the start & end…makes you think if anyone knows what’s going on

Yup I know that…but my point is when a VOSA agent tells you something you would think that your getting correct info…What if I’d done what was said then get a pull■■?
