The procedure below is for those who have lost or damaged their ADR licence. The requirement to replace a lost ADR photocard licence remains in force, as does the requirement for a driver to have their original ADR Photocard with them on board the vehicle when carrying a ‘regulated’ load.

Order a replacement ADR driver training certificate - DGDTIf your ADR Driver Training Certificates is damaged or stolen, you can have it replaced. All ADR driver training licence holders should contact SQA requesting a replacement, or access replacement form. A fee is payable by debit or credit card for the replacement.
SQA will endeavour to turn around replacements within 10 working days from receipt of form.

NOTE:
Each ADR driver training photo card certificate contains a unique Candidate Registration Number [a seven digit number at item #1 on the front of the card]. If a candidate has a problem with his/her photo card certificate, he/she should seek the assistance from the approved training provider with whom they took their ADR course.
If you need to replace your ADR Card, please don’t let any mention of the “PDP” worry you.
The PDP (Petroleum Driver’s Passport) is an extra SQA course for petroleum tanker drivers who need to enter refineries, and doesn’t affect anybody else, so the vast majority of drivers won’t need to fill in those parts of the form.
