I have a question regarding the correct completion for Form for an attestation of activities.
On guidance note 5 write:
„In order to be valid, the form must be signed both by the company representative and by the driver before the journey”
Example:
Input:
Employer’s operational centre: London;
location to take charge of a vehicle: Berlin.
departure date driver (from London): 20.01.2015;
date of arriving at location to take charge of a vehicle: Berlin : 21.01.2015.
Requirements:
correct completion of the fields nr. 20 and nr. 22 from Form for an attestation of activities:
Option 1:
At the field nr. 20 write date 20.01.2015 and place London and at the field nr. 22 write date 21.01.2015 and place Berlin.
Option 2:
At the field nr. 20 write date 20.01.2015 and place London and at the field nr. 22 write date 20.01.2015 and place London.
What is the correct answer?
I think the correct answer is option 2, followed by manual processing with the introduction of the tachograph card.
