Both cards in first thing, swap at 4.5 hrs driving or 6 hrs work unless you take 15 mins before the 6 hours work. (depends on how you and your 2nd man get on and what work you’re doing).
If you’re doing the full 20-21 hours, keep it simple (4.5 / 6hrs)
Note, its not uncommon to get an infringement on digitacho if you swap at 4.5 hrs while driver 1 (now driver 2 ) is doing nothing in the passenger seat.

Yes.
Driver 1 slot refers to the driving seat.
Driver 2 slot refers to the passenger seat.

The cards have to be swapped as the tacho will only record driving on the card in slot1 so whoever is in the driving seat should have their card in slot1, and the passenger in slot2.
The tacho head will not record break on the card in slot2 while the vehicle is moving, even though it is legal for the passenger to take a break in that situation, and so the authorities will count the first 45mins of POA recorded in such a situation as break.
On a pedantic note the limit is 4.5h of driving and not 4h, but you probably already knew that.
