Application for Tachograph smart card

Do I need to send my UK photocard Driving Licence together with D777B form (Application for a digital driver card) to DVLA when I apply for first tachograph smart card?

Or I just send D777B form alone?

just the form and the fee, mine took 1-2 weeks, as long as you have photo licence .
