Fatal forklift accident Facebook video

It’s because of incidents like this why we then all have to put up with ridiculous H&S rules. Such as being told where you can and can’t walk, when you can move, when you can and can’t speak or breathe etc.

You look at that video and yeah my sympathy is with all those involved, but surely it would be common sense to not act in the way they all were leading up to the incident.

had a safety warning notice come round yesterday , as follows

'Brief Description of the Incident
The injured Team Member place his finger in a skip hole to check for alignment with the slew base and contamination. He then operated the crane and trapped his finger.
Main Root Cause
Placing finger in the hole and mistakenly operating the crane.
Not being able to fit the pin in the holes
Corrective Actions
Do not use finger to check in such a situation
Check for contamination and alignment before task
Create a tool to check
Use stop-call-wait if there is a hazard or issue.

to sum up don’t stick finger in hole and then operate crane. easy isn’t it.

Their ineptitude in getting the truck off her prone body shows just how much more damage can be done to an already bad situation - by simply not knowing what to do.

She wasn’t moving throughout though, and could well have been killed instantly. :frowning:

Just watched it on Youtube, I’m not sure which was worse the accident or the other workers driving the forklift forward afterwards :open_mouth:

id think it was the latter that definitely finished her off…theres a chance she was just squashed till he rallied the forklift over her and squidged her ragged.he should have got off his ■■■ for a look before wellying it in a flap…funny how you can watch it on faceache,and youtube but deemed to be too terribly unfit for your average truckie to look at…id think most have seen worse during your working life passing motorway carnage.h&s in the uk is totally ott unrealistic,but at the same time,theres no excuse for being so bleeding stupid.

Knowing the weight of a forklift and the lack of ground clearance I reckon the poor lady was gone as soon as it landed.
Only consolation in this tragedy is she would not really have felt any pain.
What a nightmare for all involved though.

As much as people whinge on here about H&S rules, let’s hope none of them end up being a statistic like that. Something so daft and a quick decision can easily cost your life in places where trucks and forklifts operate.
Hence the need for over the top rules these days.
Someone at that place will be in deep do do over that, most likely the driver of the FLT as they were trying to lift something far too heavy for the machine and possibly the guy pretending to be a counterweight.
A preventable accident as always.

I think most drivers and people in general are happy to go along with health and safety rules. The benefits health and safety have had to industries like mining and chemicals is clear to see. Most people, me included that moan about it are referring more to the attitude of places using a ‘when it suits’ and basic box ticking health and safety. For example a company staff will not watch a driver back out on a road because of h&s, yet will happily let a driver reverse out on his own completely blind. I also mentioned to a firm recently that they should have a sign instructing drivers to reverse into their yard, as from the road it looks like they have a 1 way system. He said they can’t because if you crash while backing in they could be at fault!!

just seen it on you tube,not nice but you expect it in places like china or wherever it was,life is cheap in these places,but the idiots should have secured the truck and jacked it up to get her out,i can only assume for the first few seconds the forklift driver didn’t realise she was under there which is why he drove forward.
